It had "Return to Sender,No Longer Here, Cook County Hospital" stamped in red ink over the address. I'm not sure what to think of this.

Thats because you sent it Cook County Hospital. The address for the Blackhawks is 1901 W. Madison St.

It's listed as 1900 in Harvey '07.
