Found this site, that is actually kept up-to-date with the current mailing addresses of pretty much any North American Hockey team you can imagine...(no college teams...)

Beware sending c/o the Wings. For some players, they stamp on a signature and trace around it. I've gotten Zetterberg and Osgood from addresses that aren't in c/o the Wings, and have requests out to Datsyuk and Lidstrom.
